docs: generate ADR deferral index

Purpose

Accept ADR 0081 with its implementation under D-085's successor rule. Add a deterministic, derived, non-authoritative ADR-deferral index with exact source payloads and inclusive spans, fail-closed ambiguity review, explicit file-existence resolution, and a separate irregular-form review queue.

The generator deliberately ignores ADR frontmatter status as both an extraction predicate and an implementation-authorization signal. The proposed registry-to-charter pointer check is dropped; the existing self-governed authority-document carve-out remains unchanged.

• Precedence respected — no accepted ADR is relieved or overridden.
• Charter text quoted verbatim or untouched — charter text is untouched.

Charter compliance

Complies with implementation charter v1.6.0. Ring: none (Class J docs/CI tooling). No cross-ring imports added.

Required Class-J hcs-architect review completed before commit. Three design-objection rounds and two implementation-objection rounds were folded; the final review and the post-review provenance-marker delta review each returned no blocking or non-blocking objections.
