Adds the file-addressable successor table and the review-only gateway token rollup to the generated deferral index, per ADR 0081's rendering-only amendment. Rendering only — per-ADR output is byte-identical. Supersedes #105, which GitHub auto-closed when its base branch was deleted during #104's merge; commit rebased onto main, verify passing locally. Architect review on the original: no blocking or non-blocking objections.
