[codex] Sanitize public test fixtures and docs

Summary

• Sanitize public test fixtures and gateway docs so examples use neutral names instead of internal environment or client terminology.
• Replace an absolute local tool path with a project-relative example path.
• Keep the policy behavior covered by tests, using generic classification keywords.
