fix(web): remove untested server-VAD caller path (restore client VAD)#62

The server-VAD caller path (continuous streaming) broke mic input in a live test. Revert the web side to the proven client-VAD behavior so the caller mic works reliably again. Relay-side config separation + optional near_field noise reduction remain (env-gated, default off). Enable near_field via SESSION_A_NOISE_REDUCTION on the relay. NEXT_PUBLIC_RELAY_CALLER_VAD is no longer used. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>
