[codex] Ignore fctry state file

Summary

• stop tracking the generated .fctry/state.json file
• add .fctry/state.json to the root ignore rules so future local state changes stay out of Git

• inspected staged diff before commit
• no app tests run; this is a Git metadata/ignore-only cleanup
