Complete SuperLoop v2 identity migration

Summary

• rename the npm package, canonical Skill, plugin identifiers, protocol API, state roots, schemas, examples, and repository links to SuperLoop
• release the breaking identity change as 2.0.0 with superloop/v2 and .superloop/loops
• document a fail-closed v1-to-v2 migration through a parent-linked proposal revision, fresh human approval, and a new state generation
• retain legacy v1 evaluation artifacts as historical evidence and label v2 evidence honestly as a migration review
