fix: make writable discovery responsive

Summary

• remove the mobile min-content width that expanded discovery dialogs to roughly 701px inside a 390px viewport
• keep inferred INSERT/UPDATE/DELETE labels visible by stacking write metadata below each table name
• publish the uniform v0.4.1 patch contract and 0.4.0 -> 0.4.1 extension upgrade path

Scope

The data-plane write behavior is unchanged; this is a responsive control-plane fix plus the patch-version packaging contract.
