Added AR4 SPM, TS, Ch4, 6, 7, 11; including CAs for those added

In my review of AR4 entries, I noted a couple of missing chapters (including the SPM and TS)

I have

• ar4-wg-i.bib - Added all missing chapters, page numbers, and ISBN (for new Chapter entries all CAs also listed)
• Run Makefile (bibtex2html over updated *.bib files)
