Move the pre-convention planning/specs/ design doc into a proper changes/2026-06-04.01-faststream-0.7-migration/ bundle with lean frontmatter (summary only; date/slug derived from the dir name), and drop the old specs/ + empty plans/ layout dirs.

Follow-up to the convention adoption (#16): the pre-convention
planning/specs/design doc was left in the old layout. Migrate it into a properchanges/2026-06-04.01-faststream-0.7-migration/bundle (lean frontmatter:summaryonly, date/slug derived from the dir name), remove the now-emptyspecs/+plans/dirs, and drop the stalespecs/README pointer.just lint-ci+just check-planninggreen;just indexnow lists the migration.
