[codex] Lighten loop manager control-plane rules

Summary

• Add lightweight manager/dispatcher control-plane guidance for normal monitoring.
• Keep hard gates heavy for planning/execution boundaries, P0/P1 findings, required Claude gates, and worktree incidents.
• Reduce routine ledger/worklog/state-feedback duplication and make lifecycle cleanup stage-gate driven.

Validation

• Ran git diff --check locally before creating the commit.
• Verified the remote branch was created via the GitHub Git API after HTTPS git push failed in this environment.
