KIP-311: define CNPeers admission policy

• rename peerNodes terminology to CNPeers
• define inputs MaxPhysicalConnections and DialRatio
• clarify finite peerTargets semantics
• document exact static/trusted exemption behavior
• describe what changes from the existing implementation
