Fix installation handoff quiescence for HRA v0.1.13

Summary

• Exclude only the handoff operator PID from the bound main SQLite file lsof probe, while keeping WAL, SHM, journal, and bundle-root probes fail-closed.
• Add real macOS holder coverage and deterministic handoff/rollback ordering evidence.
• Advance the release candidate to v0.1.13 build 14, retain immutable v0.1.12 build 13 as a supported prior HRA, and keep tagged-only v0.1.11 invalid.
