DP-Next stands for *Sustainable Type 2 Diabetes Prevention for the 21st Century*. It is a research project conducted in collaboration between all seven Steno Diabetes Centres in Denmark, Greenland, and the Faroe Islands.

The aim of DP-Next is to improve early identification and prevention of type 2 diabetes. Today, we do not fully understand why some individuals with elevated blood sugar develop type 2 diabetes while others do not. DP-Next seeks to improve this understanding and develop more effective and sustainable prevention strategies.
The purpose of DP-Next is to improve early identification and prevention of type 2 diabetes. Some people with slightly elevated blood sugar levels develop type 2 diabetes over time, while others remain stable or return to normal blood sugar levels. Today, we still do not fully understand why this happens. Risk of type 2 diabetes is influenced not only by blood sugar levels, but also by factors related to lifestyle, health, sleep, mental wellbeing, social conditions, and biology.

The project combines:
DP-Next seeks to improve understanding of these differences and develop more effective and sustainable prevention strategies.

- Questionnaire data on health, lifestyle, and wellbeing
The project combines large-scale health data, clinical research, and development of new prevention approaches through:

The project combines different types of information to better understand diabetes risk:

  • Information from national health registers
  • A large-scale survey of approximately 30,000 participants
  • Detailed health profiling and clinical measurements from approximately 1,000 participants

The purpose, content, and structure of the survey and the clinical examinations are described in more detail below.

- Questionnaires about health, lifestyle, and wellbeing
- Information from national health registers
- A new clinical cohort with detailed health examinations
- Development and evaluation of new prevention strategies
- Clinical examinations and biological samples

Approximately 30,000 people will be invited to participate in the questionnaire study, and some participants may later be invited to additional clinical examinations.

The overall goal is to improve prevention of type 2 diabetes by identifying individuals at higher risk and developing more targeted and effective prevention approaches.
Ultimately, DP-Next aims to support more personalised and effective prevention of type 2 diabetes.

Participation is voluntary.

## What do the clinical examinations involve?

Some participants will be invited to attend a clinical examination at a Steno Diabetes Centre. The purpose is to establish a new clinical cohort with detailed health measurements to improve understanding of early biological changes related to diabetes risk.

The examination takes approximately 4 hours and may include:
The examination takes approximately 4 hours. Depending on the study site, examinations may include:

- Measurements of height, weight, and body composition
- Blood samples and blood sugar measurements
- A glucose tolerance test
- Measurement of liver fat
- Blood pressure and cardiovascular measurements
- Assessment of physical activity
- Measurement of liver fat and cardiovascular health
- Wearable devices and glucose sensors
- Storage of biological samples in a research biobank

These examinations help researchers understand differences in diabetes risk between individuals and identify early biological markers of disease development.

## What does the intervention involve?

A key goal of DP-Next is to develop new and more effective strategies to prevent type 2 diabetes.
A key goal of DP-Next is to improve prevention of type 2 diabetes through more personalised and effective approaches.

Based on findings from the questionnaires, health registers, and clinical examinations, researchers will develop prevention strategies tailored to different risk groups. These strategies are developed in collaboration with participants, healthcare professionals, and researchers to ensure they are relevant and feasible in real-world healthcare settings.
Using information from questionnaires, health registers, and clinical examinations, researchers will investigate how prevention strategies can be better tailored to different risk profiles and individual needs. These strategies are developed in collaboration with participants, healthcare professionals, and researchers to ensure they are relevant and feasible in practice.

The intervention development may focus on:

- Supporting healthy lifestyle changes
- Improving prevention and follow-up in healthcare
- Developing new approaches to prevention tailored to individual risk
- Developing more targeted approaches to prevention

The aim is to develop sustainable prevention strategies that can be implemented in practice to reduce the risk of type 2 diabetes.
