Prepare v1.28.0 release

• cut the unreleased changelog as v1.28.0 and add reviewed release highlights
• synchronize every tagged and demo module requirement to v1.28.0
• add providers/meta to module maintenance, tag publication, and provider-module CI coverage
• format the three recently merged Markdown files that did not match the pinned Prettier version

Release scope

This minor release adds the Meta Model API provider and Muse media support, makes the managed screen the experimental CLI default, persists CLI history and model settings, simplifies Bash tool results, and fixes reasoning and tool replay behavior.
