fix: require iterator PR after push

• Require the PHP transformer iterator workflow to complete through a pull request path, not issue fallback
• Expose the Data Machine Code PR creation/comment abilities and recorder explicitly in the iterator workflows
• Tighten the iterator bundle runtime rules so edit/status/commit/push/PR/comment happen in order
• Remove issue fallback from the PR-first completion path
