fix(review): keep proposal acceptance visible

Summary

• keep Accept proposal visible throughout active proposal review
• disable it with an explicit remaining-file explanation until every file has a human decision
• preserve its geometry when the final file becomes reviewed, without changing terminal authority or challenge semantics
• bump the release to 0.6.14
