Simplify compose stack to rely on external PostgreSQL

Summary

• remove the bundled PostgreSQL service from the compose stack so the app runs in a single container
• require DATABASE_URL to point at an external database and update the environment example accordingly
• refresh the README instructions to document the single-container setup and external database requirement
