fix: keep failed agent runs in review

Summary

• Port upstream cline/kanban PR fix(web-ui): keep failed agent runs in review instead of auto-completing them cline/kanban#559 into this fork's diverged web-ui paths.
• Keep failed/error sessions in Review by blocking auto-review git actions and auto-done, including delayed timer race checks.
• Render failed review sessions with a red error dot while preserving clean review success indicators and existing needs-input/credit-limit precedence.
