Agents repeatedly patched symptoms instead of the underlying cause, for example stacking five guards on one duplicate-registration error and pruning working tools on a heuristic. This skill forces a one-sentence cause statement before any fix and treats a returning symptom as proof the prior fix was symptomatic.
