executor v1.5: integrations and connections rework - #896

Target shape for the in-place migration of @executor-js/sdk. Implementations are deferred — they port from the existing v1 SDK. - 1:1 connection:integration; secret === account === connection - owner (org|user) replaces the scope stack - credential providers axis (default store + external like 1Password) - tools persisted per connection (plugin resolveTools at create/refresh) - generic plugin add() projection over the registered plugin tuple - elicitation + invoke error set (Schema, ported from v1) - whole surface on Effect; Schema-branded ids; split per-domain files Additive only (not yet wired into the build; bun.lock unchanged).
