Adds file and HTTP-backed Policy Fabric decision backends behind the existing PolicyFabricBackend protocol.
Includes JSON fixture backend, minimal HTTP GET backend, config fields for fixture/endpoint/token-env/timeout, dispatch CLI wiring, and tests for fixture, HTTP, config, and dispatch behavior.
