OPEN -> HALF_OPENrecovery behavior after cooldown expiryWhy
The modular refactor made the circuit breaker, authentication budget, and request gate independently testable. The existing suite covered the protection controller well but left two major reliability primitives mostly unexercised. This PR adds deterministic coverage without contacting BIU or launching a browser.
