sanitized names; fix _build_ref_series to use {prefix}_ref_MM columns
- index.py: pass dataset + month query params to the SVG generator
- api_routes.ts: add optional dataset/month to getUncertaintySvg
- page.tsx: point forecast/reference maps at seas5_forecast_* tables
