Identifiability in patient-level single-cell classifiers

When study design predicts disease: an identifiability limit for patient-level single-cell classifiers

Version v2.0.0-rc3 is a self-contained study of design-label identifiability in patient-level single-cell classification. The public repository and Python package retain the historical name rheumlens for backward compatibility; that name is not used as a manuscript concept or method.

The complete review object is under release_v2.0.0-rc3/. It includes the manuscript, eight editable figure groups, Supplementary Tables S1-S30, donor-level inputs and predictions, simulations, complete-pipeline nulls, two locked environments, checksums and executable validators.

Current scientific question

Single-cell RNA sequencing measures cells, while clinical labels and statistical inference concern patients. We test whether high donor-level discrimination is attributable and transportable when disease status is entangled with processing wave, collection year, sequencing quality, demographics and other recorded design variables.

The current release combines:

  • disease prediction from recorded design alone;
  • a quantitative design-adjusted label-information scale;
  • 11,200 simulations and a synthetic positive control;
  • complete-pipeline permutation and design-column-shuffle nulls;
  • recovery of batch identity from donor representations;
  • linear and nonlinear fold-contained residualisation;
  • design restriction with size- and label-matched controls;
  • an internal batch-exposure negative control;
  • attenuation-uncertainty and confound-leakage sensitivity analyses;
  • cross-batch and strict source-only cross-cohort transfer; and
  • independent-target evaluation of learned set pooling.

Current findings

  • Complete measured design predicts disease at AUC 0.953 in GSE174188 and 0.952 in GSE135779.
  • The GSE135779 complete-design AUC exceeds 1,000 complete-pipeline label permutations and leaves 20.3% design-adjusted label information.
  • Donor representations recover recorded batch identity at AUC up to 0.9997.
  • Batch provides an internal negative control: batch alone predicts disease at 0.499 and batch residualisation changes disease AUC by at most 0.009.
  • Disease discrimination largely survives restriction to observed design overlap.
  • Residualisation can remove 0.230–0.430 AUC and collapse performance to chance when the projected covariates also predict disease.
  • Cross-batch and cross-cohort transfer expose the reproducible generalisation gap.
  • Learned pooling does not repair the independent-target gap.

These results support five minimum checks: quantify design-label estimability, test the complete pipeline under null data, report representation-to-design predictability, pair residualisation with matched overlap restriction, and require fully source-fitted external validation.

v1.0.2 benchmark question

Single-cell RNA sequencing measures cells, whereas clinical classification is performed at the patient level. We test which donor representation best preserves systemic lupus erythematosus (SLE) discrimination when cohort, population, and technical context change.

The primary comparison is between:

  • mean-pooled frozen Geneformer cell embeddings;
  • source-selected highly variable gene pseudobulk;
  • source-fitted PCA pseudobulk.

All supervised analyses use donors as the independent unit. Reciprocal external transfer uses source-only feature selection, scaling, dimensionality reduction, and regularization selection.

v1.0.2 benchmark findings

  • All three representation families retain strong within-cohort donor discrimination.
  • Pseudobulk provides the stronger external-transfer representation in the evaluated SLE cohorts.
  • In the 261-donor independent target, Geneformer, HVG pseudobulk, and PCA pseudobulk achieved AUCs of 0.884, 0.919, and 0.926.
  • A shared source-internal fold sensitivity preserved the larger-target pseudobulk advantage.
  • Source-only early fusion and seven fixed distribution-preserving pooling alternatives produced no stable bidirectional improvement.
  • The pseudobulk advantage widened as source donors accumulated, while per-donor performance plateaued between 500 and 1000 cells in the evaluated Geneformer configurations.
  • Geneformer discrimination was associated with an interferon-related expression axis, but matched structured expression modules produced similar attenuation.
  • Donor-mean Geneformer embeddings occupied a low-dimensional subspace relative to sampled cell embeddings; this is a descriptive geometric result, not a causal explanation of transfer performance.

Version 1.0.2 archive

Version 1.0.2 is the submission-stage research snapshot. It adds the corrected fold-scaled internal benchmark, complete Supplementary Tables S1-S20, explicit cohort-characteristic reporting, exact Geneformer extraction details, and a calibrated interpretation of external prediction scores. The self-contained snapshot is under release_v1.0.2/:

The broader repository retains the original benchmarking package, the v1.0.0 research snapshot, tests, historical supplementary tables, and earlier fixed-split analyses for provenance.

Reproduce and validate

Validate the release object:

The release carries separate exact environment files for the locked donor-level audit and archived Geneformer extraction. The commands below reproduce the legacy package and its unit tests.

The release-specific plotting and validation scripts are under release_v1.0.0/scripts/. Several scripts use project-relative archived inputs recorded in release_v1.0.0/provenance/figure_source_index.tsv; the exact source tables are copied under release_v1.0.0/source_data/.

Scope

The results apply to the evaluated public SLE cohorts, frozen Geneformer embeddings, and fixed donor-level aggregation. They do not establish clinical diagnostic readiness, causal disease mechanisms, formal cell-count sufficiency, or general inferiority of fine-tuned or learned patient representations. Data and code availability

Large raw matrices and cell-level embedding archives are not redistributed in Git. Their provenance, hashes, and regeneration context are retained in the project records. Donor-level and plot-ready outputs needed for the public manuscript figures are included in the versioned research archive.
