Vacinacao/Procedimento/Prescricao expunham nome/codigo de catalogo e nome do usuario responsavel na listagem paginada, forcando o carregamento completo de cada relacao @manytoone por linha (ate 2 SELECTs extras por registro). Os repositorios agora sobrescrevem o findAll(Specification, Pageable) herdado com @EntityGraph sobre as relacoes to-one lidas pelo mapper.
… JWT Spring MVC despacha verbos sem handler (ex.: PUT/PATCH/DELETE nas rotas de registro clinico, que so expoem GET/POST) como um erro interno; por padrao o OncePerRequestFilter pula esse redispatch, deixando o SecurityContext vazio e fazendo o AuthorizationFilter substituir o 405 correto por 401. Refazer a autenticacao tambem no dispatch de erro preserva o status original. Tambem documenta a causa do @SuppressWarnings("unchecked") na extracao dos cargos do token (erasure na API de claims do jjwt).
