fix: harden cleanup apply resumability

• Mark DB-backed cleanup rows as applying before destructive artifact/worktree cleanup so interrupted operator runs leave visible resumable state.
• Treat applying rows as remaining work, prepend current-run resume guidance, and cap DB-backed worktree-removal apply batches to one row.
