Refactor worktree cleanup classification

Summary

• Centralizes worktree cleanup reason-code classification in WorktreeCleanupClassifier.
• Reuses the classifier for cleanup summary buckets and cleanup plan resolver rows while preserving legacy bucket aliases.
• Adds pure-PHP smoke coverage for bucket mapping, resolver eligibility, and fail-safe unknown reason handling.
