Remove deprecated TEEHR evaluation integration

Summary

Removes the EVAL feature from DataStreamCLI since ngiab-teehr is no longer maintained.

For Issue #112

Changes

• Removed the -E/--EVAL CLI option and TEEHR execution logic.
• Removed the ngiab-teehr integration/version configuration.
• Removed the TEEHR integration test workflow and status badge.
• Removed EVAL/TEEHR options from datastream_guide.
• Updated related documentation to remove references to the TEEHR integration.

Testing

• bash -n scripts/datastream
• bash -n scripts/datastream_guide
• Verified ./scripts/datastream -h no longer exposes -E/--EVAL.
• Verified no ngiab-teehr references remain.
• git diff --check
