Automated iOS substrate refresh produced by refresh-ios-substrate.yml on macos-latest (manually opened — the workflow's gh pr create failed only on a non-existent ios/substrate label; branch + binaries are intact).

Refreshes the iOS-bundled substrate Mach-O binaries to the requirements.txt pins on bump/substrate-v4-2.9.5: persist 4.0.1 / edge 1.2.0 (verify stays 4.8.0). No agent-code changes.
