Wire `contextix entities` as a CLI command

entities exists as MCP tool (src/tools/entities.ts) but not as CLI. Add it.

Acceptance criteria:

• contextix entities [--search <q>] [--type <t>] [-n <limit>]
• --json for piping
• Matches the MCP tool's filter semantics
