Focus file pane on first changed line

Instead of the file contents pane starting at line 1, focus on the first changed line.

This avoids the need to scroll down to see changes.

added in #329, as an opt-in flag rather than a change to the default.

--start-at-change puts the cursor on the first changed line when a file loads, and reapplies on every file switch and on reload, so n/p through a tree of files opens each one on its change. There is also REVDIFF_START_AT_CHANGE and a start-at-change key in ~/.config/revdiff/config if you want it on permanently.

it is off by default because the full-file view is deliberate here, revdiff is meant for judging a change against the surrounding source rather than reading isolated hunks. --compact already gave you changes-at-the-top by shrinking the diff to a few lines of context; what your request adds is the same starting position while keeping the whole file, which the existing [/] hunk navigation did not do on load.
