Audit and remediate historical pull-request review feedback

Objective

Audit every review thread on all closed or merged pull requests and remediate any valid feedback that was resolved or left unresolved without an implementation.

Scope

• classify all 484 historical review threads as fixed, superseded, not applicable, or still actionable
• verify classifications against current main, not thread resolution state
• fix actionable security, correctness, privacy, performance, documentation, SDK, and frontend findings
• link each remediation to the original review comment
• resolve threads only after the correction or evidence-backed disposition is recorded

Merge gate going forward

• inspect all PR review threads after CI and immediately before merge
• implement valid feedback and rerun relevant checks
• document evidence when a suggestion is not applicable
• do not merge with unresolved substantive feedback

Acceptance checks

• all closed-PR review threads have an evidence-backed disposition
• all actionable findings are fixed and tested
• no substantive review thread remains unresolved on the remediation PRs
• the review-before-merge gate is documented in the repository

Historical review audit update:

• Merged PR Remediate maker-checker review findings from PR #250 #284 (commit d7c6145) after all CI jobs passed and a final review-thread check returned zero reviews and zero threads.
• PR Remediate maker-checker review findings from PR #250 #284 fixes all eight findings from PR Feat/ic 053 maker checker decisions #250 plus the GraphQL decision-node finding from PR Feat/ic 052 reviewer scope enforcement #248.
• Resolved exactly those nine original threads after the correction reached main.
• Local verification included 342 Django tests, 46 focused tests, Python and JavaScript SDK tests, dashboard ESLint and TypeScript, migration drift, generated SDK checks, OpenAPI parsing, Ruff, Prettier, and diff checks. Repository CI additionally passed Java, .NET, browser E2E, Docker, security, live SDK compatibility, and container jobs.

Additional evidence audit completed against current main:

• PR Feat/ic 051 versioned decision contract #249: 7 of 7 threads remain actionable.
• PR Feat/ic 052 reviewer scope enforcement #248: 6 threads remain actionable; custom-provider dispatch and timeout semantics were already fixed, and the GraphQL thread is now fixed by Remediate maker-checker review findings from PR #250 #284.
• PR Feat/ic 032 secure http provider adapter #247: 5 threads remain actionable; Pillow dependency and timeout semantics are already fixed.
• PR Feat/ic 029 provider capability contract #246: 5 threads remain actionable; Pillow dependency is already fixed.
• PR feat: make upload completion idempotent #245: 4 of 4 threads remain actionable.
• PR feat: make upload completion idempotent #244: 3 of 3 threads remain actionable.
• PR feat: bind verification sessions to scoped actions #243: 2 of 2 threads remain actionable.
• PR Feat/ic 021 immutable verification snapshots #242: 6 of 6 threads remain actionable.
• PR feat: enforce atomic verification state transitions #241: 6 of 6 threads remain actionable.

Resolution state was not used as completion evidence. The next remediation slices will group only technically overlapping findings and link every original thread.

• PR Fix session scope and workflow snapshot review findings #285 (commit 7c83c6b) fixed all 8 actionable findings from PRs Feat/ic 021 immutable verification snapshots #242 and feat: bind verification sessions to scoped actions #243: deny-by-default empty action scopes, mobile handoff scope preservation, audit/PDF workflow lineage, legacy snapshot backfill, and immutable workflow naming across REST and GraphQL. CI passed; pre-ready and post-ready review queries returned zero reviews and zero threads.
• PR Fix provider contract and media parser review findings #286 (commit 18f32ab) fixed 10 provider-contract and media-parser findings across PRs Feat/ic 029 provider capability contract #246–Feat/ic 051 versioned decision contract #249: missing provider status rejection, complete provider envelope retention, risk-signal persistence, explicit legacy decision provenance, and safe parser/decompression-bomb quarantine. CI passed; pre-ready and post-ready review queries returned zero reviews and zero threads.
• PR Finalize and scan direct uploads before evidence submission #287 (commit 5664aaa) fixed 12 direct-upload and transfer findings across PRs feat: make upload completion idempotent #244–Feat/ic 051 versioned decision contract #249: authenticated direct-upload completion, bounded reads, scan-before-consume, immutable validated storage keys, early multipart size rejection, object-storage I/O outside database transactions, consumed/promoted retry idempotency, and protection of validated state from malicious retries. The public OpenAPI contract, generated SDK models, runtime API catalog, portal client, and E2E fixture were updated. Local verification passed 354 Django tests plus Ruff, generated-model drift, portal TypeScript, Prettier, OpenAPI parsing, and diff checks. Every CI, container, dependency/secret, live SDK, and GitGuardian check passed. Final pre-ready and post-ready queries found zero general comments, reviews, inline comments, or review threads.

PR #287 intentionally did not claim the structural video-container or provider DNS/deadline findings; those remain actionable for separate slices. Issue #281 remains open until every historical thread has an evidence-backed disposition and the repository merge gate is documented.
