Advance merged foundation capability truth states

Goal

After PR #257 merged, update the capability ledger and its regression fixtures from draft-PR state to the truthful merged-repository state.

Done criteria

• Ledger no longer points at pull/257 as draft evidence.
• Specification/repository states match the documented truth-state vocabulary and the merged commit 03b5263e3b273ea7a3e76581fd73c6059fd90ada.
• Verification/activation/authority remain conservative; merge does not imply live activation.
• Focused/full tests and independent review pass.

No runtime, install, process, credential, Vault, or activation changes. Coordination: levineam/clawd#1249.
