Extend storybook with examples from case-study-record-manager-distribution

There is example3 and example4 available at ulcheyev/case-study-record-manager-deployment#43.

It would be nice to have in storybook some kind of provider of media (or deploy media to some server) and show it as new forms.

A/C:

• 2 new forms are deployed
• the roles are correctly set up for the example3 and example4 forms ... the scenarios what to do with those forms were described in Usability Testing of Learning Support Features in Record Manager
• providing the media content is stable for the storybook (options: public resource / we deploy to kbss / we provide by storybook)
