Add Assessment Form

As a student, I want to add an assessment to the study planner, so that I can work out how long I need to spend studying for it.

Requirements

• Student can add assessment consisting of:
  • Date
  • Name
  • Type of assessment (Coursework/Exam)
  • Importance/Amount of Work (1-5 scale)
• Assessment is saved to database
