Create a single Repo for the Spec

Right now the specification is wrapped up in the "mcp-reference-server" repo.

Let's separate it out so that we have all the specifications defined in one repo, then the "reference server" repo is offered as just that - a reference and a resource for helping teams understand how the system is deployed.
