Normalize method parameters

Hi,

JvmMethod.parameters() includes parameters generated at compile time

• for inner class add Outer this$0 before other param in constructors
• for Enum add String name, int ordinal before other param in constructors

We should include these in AstMethod.parameters()

ParamModel should also implements WithMod (and add isSynthetic, isMandate)
