Reduce accepted code-health debt

Track intentional follow-up reductions after the Fleet code-health gate is established. Baselines must be measured from the adoption branch and CI must reject regressions. Improvements should lower coverage gaps, legacy complexity/duplication, accepted dependency advisories, or other explicitly recorded exceptions without weakening the gate.

Accepted debt is now regression-gated: 438 tests; 55.64% lines, 47.37% branches, 63.35% functions, 53.42% statements; 81 complexity violations (max 49/615/9); 47 clone groups / 750 lines / 2.1454%; 13 suppression markers; 21 accepted high advisory IDs across 29 path findings; one exact test-contract export and one exact production dependency exception, with zero other Knip findings or cycles.
